Sending us an Email
You also may decide to send us personally-identifying information, for example, in an electronic mail message containing a question or comment, or by filling out a Web form that provides us this information. We use personally identifying information from email primarily to respond to your requests. We may forward your email to other employees who are better able to answer your questions. We may also use your email to contact you in the future about our programs that may be of interest.
We want to be very clear: We will not obtain personal identifying information about you when you visit our site, unless you choose to provide such information to us. Providing such information is strictly voluntary. Except as might be required by law, we do not share any information we receive with any outside parties. If you sign up for one of our email lists, we will only send you the kinds of information you have requested. We do not share your name or email address with any outside parties.
Bills are due within 30 days from the first of each month. A late penalty charge of 1.5 percent will be assessed on payments not received by the due date and on any balance exceeding 30 days. You will receive a statement for services on a monthly basis. It may be paid in via credit card through our website at any time.
Returned checks/declined credit cards: When a check is returned to VitalRads due to insufficient funds, the customer will be charged a $35 fee. VitalRads does not honor any post-dated checks. All checks are processed on the day of receipt.
Late Payment: When a payment is not made within one full billing period from the date of the bill, the account will be classified as past due. A late payment charge of 1.5 percent per month, translating to 18 percent a year, will be added to all amounts billed.
Clients with past due balances exceeding 60 days will be notified in writing and are payable within 10 days. This is the only notification that will be issued. If payment is not made within the 10-day period, VitalRads staff will refer the account for collection. A $40 collection fee will then be added to the amount due and service and access to the case manager will be discontinued.
Reconnection Procedure: Following service cut-off for non-payment, service will not be reinstated until payment is made and the client contacts VitalRads billing department at firstname.lastname@example.org.
